What Will You Do as a Clinical Liaison?

Connect with New Patients

Interact and build trust with our growing patient base. You will call patients for their scheduled ExactCare appointment to support them with their medication management.

Gather Patient Information

Create health and medication profiles for patients. You will review their medications, health and prescriber information, in addition to identifying social determinants of health challenges.

Explain Our Services

Explain our medication adherence program to patients and walk them through enrollment. You’ll be ensuring patients are prepared to onboard with ExactCare.

About You

  • You are a Certified Pharmacy Technician (CPhT), a licensed health care professional (such as a Licensed Practical Nurse or LPN), or a skilled professional (case manager/social worker) who wants a meaningful job helping others.
  • You care about others and use soft skills to empathize, communicate and understand other people’s needs.
  • You take ownership of your work and can effectively manage your time.
  • You have high attention to detail to handle multiple projects simultaneously and complete them timely and right.
  • You are driven to achieve goals and work well with team members to give our patients the best experience.
  • Being bilingual is a plus!
