You identify patients who may benefit from ExactCare’s services and contact us via phone or online to refer them. All we need for each patient is his or her name, address and phone number. We’ll take care of the rest!

HOME ASSESSMENT

HOME ASSESSMENT

An ExactCare representative will visit the patient at home to gather prescription information and provide an overview of how our service works. This is the first step in the medication review and reconciliation process. The information is then sent to the pharmacy for a comprehensive clinical review and to begin the process of verifying and obtaining the patient’s prescriptions.

Our clinical pharmacists perform a thorough review of the patient’s complete medication profile. We review doses for accuracy and check for things such as duplicate therapies, contraindications, and potential drug interactions. If we detect any issues, our pharmacists work with the patient’s physicians to find a solution.

THE EXACTPACK®

THE EXACTPACK®

Patients receive a customized ExactPack each month, which consolidates medication doses based on the day and time they should be taken. Every ExactPack pouch is clearly labeled with the proper administration time, a list of the medications inside and a description of each.

ONGOING REFILL MANAGEMENT

ONGOING REFILL MANAGEMENT

Our pharmacists work directly with patients’ physicians to update prescriptions and secure refills. We can even notify patients if an appointment is needed before we can obtain a new prescription.

MONTHLY HOME DELIVERY

MONTHLY HOME DELIVERY

The patient receives a delivery at home each month complete with his or her customized ExactPack, as well as any medication that may not go into the ExactPack. This may include topicals, inhalers, injectables and diabetic supplies.

Quality & Compliance

Quality checks in our processes

ExactCare utilizes several quality and safety measures to ensure patients receive the proper medication. Not only is every order checked three times by a pharmacist before it leaves our building, but we’ve also invested in advanced technology to ensure accuracy.

A key component of this process is a machine that uses digital recognition technology to individually scan each pouch in every ExactPack, ensuring it contains the proper medication. If it flags something for review, a pharmacist will examine that pouch to verify its contents. If needed, we will re-pack the ExactPack and send it through the same quality assurance process before it is shipped to the patient.
