ExactCare Pharmacy strives for the highest standard of quality assurance in the delivery of pharmacological management. Our goal is to provide continuity of care and help streamline the pharmacy process for healthcare professionals and our mutual patients. We maintain the most current and timely medication records for each of our patients to ensure patient compliance.

Available below are blank forms to print for any new medications, lab requests, and diabetic CMN. Please have the prescriber sign and fax them to 216-369-2201, and we will take care of the rest.

For maintenance therapy, ExactCare Pharmacy will fax a preprinted medication request in advance to avoid a routine prescription from not being filled on time.
